Job Description:

Seeking a PRN Nurse Practitioner for home risk assessments, including adult and geriatric evaluations, in Tyler, TX.
New grads welcome; flexible schedule including days, evenings, nights, and weekends.
Conduct up to 1-hour visits reviewing health, history, medications, and social factors to improve care and identify undiagnosed conditions.
Collaborate with physicians, social workers, and caregivers, and provide follow-up and referrals.
Requirements include a Master--------s or equivalent experience, current RN and NP licensure with prescriptive authority, certification (AANP, ANCC, or AACCN), and minimum 1-year experience.
Travel to patient homes, SNFs, and community environments with valid driver--------s license.
Compensation ranges from $2,000 to $10,000/month based on assessments completed. Benefits include mileage reimbursement and some voluntary benefits.
