#CA-DS The Senior Pharmacist is an advanced practitioner in a specialized area of pharmacy practice: Specialty Care. The pharmacist is essential to providing patient care to all specialty care patients at UC Davis Health and in the development of effective, professional relationships within the multidisciplinary team. Pharmacists share responsibility for comprehensive medication management, evaluation of complex patients with co-morbid conditions, complex medication therapies, the oversight and development of treatment guidelines and formulary management, stewardship of high-cost drug therapy, clinical research, and strategic planning and key initiatives related to pharmaceutical care.

The Senior Pharmacist is a valued clinical provider and will act as a resource to other staff members, as well as engage in the teaching and mentoring of pharmacy students, residents and staff. The pharmacist will also participate in Department and Hospital Committees, task forces, and medical staff meetings as requested. The Senior Specialty Pharmacist works to facilitate comprehensive medication management and oversight for patients with chronic diseases including but not limited to comprehensive medication management in their respective specialty area (e.g., Pulmonology, Transplant, etc.), as well as other chronic conditions seen in ambulatory care, acute care, and any applicable transitions of care. Activities include participation in daily patient visits, telephonic follow-up, in-services to physicians, assisting with cost-containment and medication approvals and educating other pharmacy staff. The Senior Pharmacist will actively participate in quality improvement projects, policy development, the documentation and reporting of clinical activities, and facilitate organizational surveys, assuring compliance with all Federal and State laws and regulations in addition to hospital policies.Apply By Date6/13/2025 at 11:59pmMinimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position.

California Pharmacist License eligible and possess a valid license at the time of hireAmerican Heart Association (AHA) Basic Life Support (BLS) CPR Certification or UC Davis Health CPRDoctor of Pharmacy Degree (PharmD) or equivalent trainingPGY-1 and PGY2 pharmacy practice residency OR five (5) years of clinical pharmacy experience (inpatient or ambulatory clinic) (minimum of 5 years in lieu of PGY2 residency training)1 or more years of inpatient experienceEducational experience and ability to work with and direct health care professionals.Ability to communicate well and make independent decisions to resolve minor problems as they arise.Complete working knowledge of, and skill to apply, practices, principles and techniques of all aspects of patient care related to comprehensive medication management and chronic disease management.Must be able to accurately perform pharmacy calculations.Ability to obtain information from a patient or patient's chart to create a management plan.Ability to effectively communicate patient care plans to healthcare professionals.Familiarity with relevant clinical practice and regulatory guidelines related to the area of specialty practice and care of chronic disease management patients.Preferred Qualifications
Pharmacist Board Certification (e.g. BCPS, BCACP); certification in specialty area is preferred if currently available. 3 or more years of inpatient experience Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English.Problem-solving skills to independently recognize and define a problem; identify the resources available to help solve the problem; create and implement viable solutions; and follow through to ensure the problem is resolved to the satisfaction of all parties.Decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached.Ability to supervise technicians and establish priorities of workflow.Key Responsibilities

The Department of Pharmacy is a dynamic, integral, and critical part of patient care provided by UC Davis Health. We also have the largest pharmacy residency program in the country with 46 total residents in 13 different programs.

The Department of Pharmacy offers comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, and teaching) throughout the health system complex 24 hours a day, 7 days a week. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations.
