The Home Health Physical Therapist will be responsible for delivering physical therapy services to patients in their homes. Daily tasks include assessing patient conditions, developing treatment plans, administering physical therapy treatments, and monitoring patient progress. The physical therapist will collaborate with other healthcare professionals to ensure comprehensive patient care and rehabilitation. Responsibilities also involve educating patients and their families on proper care, safety, and therapeutic exercises to improve functionality and quality of life. Qualifications Expertise in Physical Therapy and Rehabilitation Strong skills in Medicine and Patient Care Experience with Home Care services Excellent communication and interpersonal skills Ability to work independently and remotely Current state licensure as a Physical Therapist Bachelor's or advanced degree in Physical Therapy or related field Experience in providing home health care is preferred
