LanceSoft is seeking a travel nurse RN Care Manager for a travel nursing job in Oakland, California. Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Start Date: 03/02/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Shift - Day Shift: 5x8 hours - Hours: 08:00 AM – 05:00 PM - Schedule: Every other weekend required - Float: May float to other UCSF facilities (within 75 mile radius) Responsibilities - Manage pediatric patients’ care plans from admission through discharge - Coordinate care with interdisciplinary teams (physicians, therapists, social workers, etc.) - Perform discharge planning and arrange post?acute services - Conduct utilization review and ensure appropriate level of care - Work with families on education, resources, and transition of care - Navigate complex cases in NICU/PICU and general pediatrics - Document all case management activities in the hospital’s charting system - Ensure safe, timely, and effective patient transitions Job Requirements & Qualifications Licenses & Certifications CA RN License (Required) AHA BLS (Required) COVID?19 Vaccine Booster required (Medical/Religious exemptions only) Flu Vaccine: Unknown Experience Requirements 2 years Case Management experience (Required) Discharge planning experience required Travel nursing experience: Required (within the last 12 months) Pediatric Case Management experience: Required NICU / PICU experience: Required Trauma Level II experience: Required Trauma Level I experience: Preferred Community Hospital experience: Preferred Teaching Hospital experience: Preferred Education BSN required Master’s in Nursing or Healthcare-related field: Preferred About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
