Job Description A home-health hospice facility based out of
Modesto, CA is seeking an experienced Physical Therapist to join
their team! Physical Therapist Salary: $125,000 - $145,000/annually
Physical Therapist Opportunity: Full-time, direct hire Schedule:
Variable; seeing between 2-8 patients a day Setting: Home Health
Assess and develop care plans : Conduct comprehensive evaluations
of patients' physical abilities and design individualized treatment
plans in collaboration with the interdisciplinary care team
Implement interventions : Provide therapeutic exercises and
mobility training to maintain or improve function, comfort, and
quality of life for patients with terminal illnesses Educate and
support caregivers : Train family members or caregivers on proper
body mechanics, safe transfer techniques, and ways to assist with
daily activities to ensure patient comfort and safety at home
Physical Therapist Qualifications: A Doctor of Physical Therapy
(DPT) degree from an accredited program. A license to practice
Physical Therapy in California At least 1 year of Physical therapy
experience required Experience in home health setting is a plus
