Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 03/30/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Summary Position: Registered Nurse – Case Manager (Home Health) Location: Novato, CA Contract Duration: 13 Weeks Start Date: 03/30/2026 Shift: Day Shift – 5x8 Hours (08:00 AM – 05:00 PM) Unit: Case Manager RN – Home Health Requirements: - Active California RN License - BLS (AHA or American Red Cross) - Minimum 2 years RN experience - Recent Home Health Case Manager experience required (NOT Hospice) - Epic charting experience required - OASIS documentation experience required - Valid Driver’s License (field visits required) Work Setting: - Home Health Case Management - Patient visits within 35-mile radius of home office Unit Details: - Average Visits: 5 per day - Patient Population: Adults & Geriatrics - Daily Census: 345 - Weekend Coverage: Once per month Responsibilities: - Manage a caseload of home health patients - Coordinate interdisciplinary care plans - Conduct in-home patient assessments and education - Ensure CMS compliance and accurate OASIS documentation - Collaborate with physicians, therapists, and social services to maintain continuity of care About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
